Welcome to the Diabetes Initiative
The Diabetes Initiative is a national program of the Robert Wood Johnson Foundation. It focuses on Advancing Diabetes Self Management and Building Community Supports for Diabetes Care in real world clinic and community settings of diverse populations. The Initiative also includes a Childhood Obesity Prevention pilot project in four sites that focuses on innovative policy changes to promote healthy eating and physical activity in family, school and community environments.
