Mission
The mission of the Diabetes Initiative is to improve the health and quality of life of people with diabetes through advancing diabetes self management systems and services in primary care and community settings.

Leadership
The Diabetes Initiative National Program Office (NPO), a national program funded by the Robert Wood Johnson Foundation is housed in the Division of Health Behavior Research at the Washington University School of Medicine in St. Louis, Missouri. The NPO works in conjunction with its program director at the University of North Carolina at Chapel Hill School of Public Health.

The NPO serves as the coordinating center for two diabetes self management programs: Advancing Diabetes Self Management and Building Community Supports for Diabetes Care. These two programs support 14 demonstration projects that test and introduce quality improvements in the resources and supports for diabetes self management in primary care and community settings, respectively. The NPO facilitates learning and quality improvement among the grantees through the use of a collaborative learning network, workgroups, expert consultation and focused training.

The NPO also coordinates a Childhood Obesity Prevention pilot project that focuses on innovative policy changes that promote healthy eating and physical activity in family, school and community environments to reduce the risk for obesity and development of diabetes in children.

The NPO works closely with leadership from the Robert Wood Johnson Foundation, our National Advisory Committee, and our external cross site evaluation team at RTI International.
