Assessment of Primary Care Resources and Supports for Chronic Disease Self Management (PCRS)

The PCRS tool was developed by the Diabetes Initiative to further define the elements of self management support in primary care. Its purpose is to facilitate quality improvement in this component of the Chronic Care Model. [download tool]. They can be used by both teams and management to guide, record and monitor improvement processes. The first is a PDSA-style worksheet adapted for use with the PCRS [download worksheet]*, and the second is a companion summary report. [download summary report]*

Building the Business Case for Diabetes Self Management: A Handbook for Program Managers

This handbook was conceived to meet the need to assemble in one place the rationale, methods and tools for building a business case for self management of diabetes.

--Chapter 1 provides the rationale for establishing a business case for implementing quality-enhancing interventions in diabetes, discusses procedural steps and methodological issues that must be addressed in developing a business case, and suggests conditions that should be met for an organization to embark on a successful business case analysis.

--Chapter 2 addresses the nonfinancial business case in greater depth. It introduces an expanded framework for the business case, taking into account other perspectives such as those of customers, communities and regulators.

--Chapter 3 presents the Microsoft Excel® spreadsheet templates for computing the return on investment for quality-enhancing interventions  in a variety of settings. [download templates] [download sample]

--Chapter 4 provides a brief review of the evidence of the cost-effectiveness of self-management support in diabetes. The handbook ends with a bibliography that expands on the chapter references. [download handbook]

Tools for Building Clinic-Community Partnerships to Support Chronic Disease Control and Prevention

These tools are organized around a Framework for Building Clinic-Community Partnerships to Support Chronic Disease Control and Prevention.  The Framework outlines how essential partnership characteristics build capacity necessary to achieve specified intermediate and long term outcomes. [download Framework].It was developed by the Building Community Supports for Diabetes Care (BCS) Program of the Diabetes Initiative. BCS projects demonstrated the key role clinic-community partnerships play in supporting diabetes care and healthy self management. The Tools include three self-assessment checklists that correspond to the first three phases of the framework.  The checklists are designed to help partnerships track the progression of their work, facilitate discussion among partners, and identify areas for improvement. The last tool, Taking Action—Making Improvement, was developed to help partnerships move from assessment to action.  This tool poses questions to the partnership that are intended to help them initiate plans to address areas identified for improvement.
