This report presents the approach to providing technical assistance to grantees of the Diabetes Initiative. The report describes the collaboration and synergy among grantees, program staff and advisors that led to program evolution and improvement. It also discusses grantee perceptions

Sustainability: a Retrospective Assessment of Diabetes Initiative Projects

One of the goals of the Diabetes Initiative  grantees was to develop and implement strategies that would allow them to sustain their programs when grant funding ended (after 45 months). This report details grantee experience with successful sustainability strategies, identifies threats to sustainability and discusses key ingredients for success in sustaining programs.
