Advancing Diabetes Self Management
The Advancing Diabetes Self Management program supports projects to improve self management supports through organizational and programmatic changes in primary care settings. Linkages to effective community resources and community-based programs are recognized as important components of diabetes self management.

   

Building Community Supports for Diabetes Care
The Building Community Supports for Diabetes Care program funds innovative community-clinic partnerships aimed at supporting and improving diabetes self management among high-risk populations.

   
Childhood Obesity Prevention Pilot Project
The Childhood Obesity Prevention pilot project of the Diabetes Initiative focuses on innovative policy changes that promote healthy eating and physical activity in family, school and community environments to reduce the risk for obesity and the development of diabetes in children ages 3-12 years.
