Summary

Self Management Goal Quality Poster (PDF)

This tool is designed to help health providers assist diabetes patients in making high quality self management goals. High quality goals are scored a total of five points. One for stating the activity, one for stating duration, one for frequency, one for location, and one for self assessment of likelihood of success. An example high quality goal might be, "I will walk (activity) on a treadmill at home (location) on M-W-F at 6 a.m. (frequency) for 30 minutes (duration). On a scale of 1-10, my likelihood of success for this goal is an 8 (likelihood of success)."
