BCS Community-Based Organization Questionnaire (PDF)

This three-page interview script is designed to help learn about the needs in the community around diabetes from community-based organizations and health centers. It can also help identify potential partners. It was tailored to be used with the Open Door Health Center in the Homestead/Florida City area, but can be altered to fix other communities.
